Dental Insurance Company Denying Authorization for Covered Plan Services.?

Anthem Blue Cross Dental Net I was referred to an oral surgeon by my primary care dentist since I have to have 6 teeth and 2 residual root tips extracted in advance of getting a full upper denture. All the extractions including the root tips have to be done during one visit, which necessitates the use of IV sedation. My general dentist cannot perform any of this work. Anthem Blue Cross Dental Net denied the original pre-authorization request, and subsequent appeal stating that my general dentist can perform the services being referred. Which is absolutely not the case. I even spoke with a customer service rep who indicated that the extraction of residual root tips is definitely a referable service and it clearly states that in their Dental Net Provider Contract. She has no explanation for why the appeal was denied. My primary care dentist has even provided Anthem Blue Cross Dental Net with his specific reasons for referring this work to an oral surgeon and I still can't get anyone at Anthem Blue Cross to re-evaluate the original denial and the appeal and authorize the necessary work. I have filed a complaint with the California Department of Insurance, although I don't hold out much hope for any results from that effort. HELP... what are my options? Who can I contact? Thanks

So I've been looking around the internet for a cheap car to insure for my first car. The only ones I seem to find are newer cars which are around the 3000 mark to buy. Right havent passed yet but been looking as I had last year, last year the cheapest I found was with Quinn-insurance on a Seat Arosa 1.0 2000 3DR, which was something like 700 for Third Party F&T. First time, on a 2002 BMW M3 I got quoted $140/mo for full coverage (I'm 18, completely clean record), went back through to see if I could get the same result: got $199/mo, same information and everything. So about a month and a half ago, someone kicked my car at night and left a big dent. I was stupid and called insurance and they gave me two options: fix the car or take a cash settlement. However, no matter what I do, my monthly insurance rate is going to go up even though it was not my fault. I think this is ridiculous and not worth it and I just want to cancel the claim. But I have been told that once a claim is put in, there's nothing you can do and once you hit three claims, your overall premium goes way up. What do I do? Should I just stop contacting the insurance company and after a while they have to close the claim? I wanted to sell the car this week but I guess that adds too many complications...

I've just been told that Mega Health and Life Insurance was a rip off company. (After I've pretty much wasted my time giving them information). Humana is so darn expensive for me. I'm a single mother of two. Working from home, due to my mystery illness. As of now I have no health insurance, but get by/ struggle with Care Link. ( Basically that allows me to make low payments on my medical bills, instead of paying the full amount up front) But I feel because of this method, most doctors aren't being up front with me. I also think this limits me from being honestly tested and checked out for my current on going problems. (I think they see me as a poor person that would never pay the bill off, so why test me?)
